Establishing Care
All initial or new patients must call (845) 480-5406 to get an appointment and then a link to our patient portal will be sent to start the intake process. After reviewing the intake forms, someone from our office will reach out to you to discuss whether we are able to meet your needs.
getting the most out of your appointment
Whether meeting in-person or via telehealth, patients should complete the routine pre-visit questionnaire, have a list of all current medications and supplements, and please comply with the contract for controlled substances.

Write down any new symptoms you’re experiencing, any questions you have, and changes to your medical history.
for Telehealth visits

Prepare for the visit beforehand by ensuring your technology is working properly.

Ensure that you are situated in a quiet place where you can visit with Dr. Odetalla without interruption.
